Board and Commission Application Instructions

This electronic application form can be used in one of two ways:

You can simply print the form, fill it out, and return it to the Clerk's Office. The "Print Form" buttons at the bottom of each page will bring up the print dialogue box.


You can complete the form online using Acrobat Reader, print out the completed form, sign it, and return it to the Clerk's Office. When filling out the form electronically, use the Tab key to move between fields, and the "Reset Form" buttons to clear the fields on the corresponding pages. When you are ready to print, the "Print Form" buttons will bring up the print dialogue box.

Once the Clerk's Office receives your completed application, we will forward the application to the chairperson and staff liaison of the appropriate board. If you would like to know the status of your application, you may call the Clerk's Office at 337-8791 for more information.
